Release of Information

A release of information form is a form signed by an individual authorizing a specified agency to give  speficic information about the individuals situation and/or needs. A Consent Form must be used to obtain permission to share information with other providers.

 

Complete this form only if you want us to give information or records about you, a minor, or a legally incompetent adult, to an individual or group (for example, a doctor or an insurance company).
